Initially presenting with abnormal eye movements and macrocephaly, the patient progressed to severe hydrocephalus caused by aqueduct compression from a giant dural venous sinus with thrombosis and shunting.

Johanna Fifi details a sophisticated staged intervention: the placement of an external ventricular drain, followed by two endovascular embolization procedures using Onyx liquid embolic and balloon-assisted techniques targeting meningeal feeders.
