Ming YE (Beijing) describes a challenging case of a complex middle cerebral artery (MCA) aneurysm in a 74-year-old woman. The patient was incidentally diagnosed with unruptured bilateral MCA aneurysms and classified as having multiple aneurysms.

After imaging and investigations, the surgical team determined a high risk of rupture. They opted for staged treatment using microsurgical clipping, prioritizing the left lesion, which was deemed more dangerous than the right.
