Y-Stent-assisted coiling for Left MCA bifurcation aneurysm

Gaurav Goel and his team at Medanta Hospital (India) performed a live case demonstrating the successful treatment of a complex, wide-necked left middle cerebral artery (MCA) bifurcation aneurysm. The patient, a 51-year-old male presenting with a severe holocranial headache, had previously undergone coiling for a ruptured DACA aneurysm. This session highlights step-by-step technical strategies for managing challenging bifurcation anatomies using a dual open-cell stent construct.

Technical execution and Y-Stent construction

To reconstruct the MCA bifurcation and secure optimal neck coverage, operators selected a Y-Stent-assisted coiling strategy. Using an 0.71-inch Polaris guiding catheter and SL-10 microcatheter, 3x21 mm Neuroform Atlas stents were sequentially deployed from both the superior and inferior divisions into the main stem, paired with target coils.
