This live case from St. Michael’s Hospital demonstrates the endovascular management of an incidental, unruptured 10 mm saccular aneurysm located at the left ICA paraophthalmic segment in a 52-year-old patient.

To optimize procedural safety, the clinical team used advanced simulation to plan their treatment strategy. They selected a SURPASS ELITE 4.5x20mm flow diverter for proximal deployment. Crucially, the procedure was performed under neuro-optical coherence tomography (OCT) imaging control. This high-resolution intravascular imaging modality allowed the operators to precisely visualize the underlying vasculature, intraluminal objects, and the subarachnoid space, providing real-time evaluation of the device-to-wall apposition.
