Recurrent previously clipped aneurysm ACOM aneurysm treated by bilateral flow-diverter reconstruction of ACAs; a first malpositionned FD is removed with a lasso

The patient is a 45-year-old woman with a history of a ruptured ACom aneurysm that was clipped in 2003. In 2020, she presented with a sudden onset of headache. The angio-CT scan didn't show any aneurysm but revealed some artifacts from the surgical clip. Further investigations confirmed a recurrence of the ACom aneurysm.

The surgical team performed bilateral flow diverter reconstruction of the ACAs and also had to remove a first malpositioned flow diverter with a lasso.
