The patient is a 54-year-old woman with a history of left internal carotid dissection in 2012 leading to ischemic stroke and mild expressive aphasia. After she fell from her bicycle, she underwent a CT scan that showed a major subarachnoid hemorrhage. During her transfer for angiography, imaging revealed a ruptured dysplastic wide-neck basilar tip aneurysm, as well as a dysplastic AICA aneurysm.
