After 3 weeks of persistent fever initially treated as prostatitis, the patient presented with transient aphasia. Blood cultures confirmed Streptococcus Bovis, and imaging revealed subarachnoid hemorrhage (SAH) and left parietal ischemia, likely due to an infective embolus. Following initial antibiotic therapy, a follow-up angiography showed the development of a mycotic aneurysm, requiring critical decision-making on the next treatment step.
