A middle-aged male patient presented to the ER with brutal headaches, revealing a large left MCA aneurysm treated with coils. An aneurysm recurrence was present at the 6-months follow-up. This recorded case presented the importance of balloon testing before flow-diverter stenting in the MCA bifurcation.
