Third stroke patient treated with Cyclical Aspiration

Discover how Vladimir Kalousek from Croatia treated his third stroke patient with Cyclical Aspiration (using the CLEAR Aspiration System, Insera Therapeutics).

Case

 

  • Male patient with onset of stroke after during the morning – posterior circulation, mild to severe paresis of the right extremities, dysarthria, tongue deviation to the right and internuclear ophthalmoplegia
  • Transfer from an outside hospital where they performed DSA which was interpreted as stenosis of the middle part of basilar artery

 

First DSA

 

  • It was decided to do another DSA to better determine the aspect of stenosis or if it is real stenosis or maybe fresh cloth
  • Rightside femoral approach, GA
  • Fubuki 7F into the left vertebral artery, sofia 5f
  • Gentle maneuver with wire / traxaccess 14 and headway 21 passed with ease the narrow part of the basilar artery which we interpreted as a fresh thrombus and deployed catchview 20 into basilar while sofia 5f was connected to the cyclic pump and brought onto catchview

 

 

  • Control run showed TICI 3 score, without signs of stenosis
  • During next morning he started to recover with better motoric strength on the right side, as well as speech
  • Control CT showed minor infarction in the left cerebellum

Conclusion on small series of performed cases showed positive experience with the cyclic pump so far, and some advantages as possibilities of creating own setup of cyclic motions with variable pressure.
