Discover the case of a 91‑year‑old patient initially admitted to the respirology department for worsening shortness of breath associated with bilateral pleural effusion. During hospitalization, she developed right-sided weakness and muteness.

Her medical history was significant, including chronic left-sided pleural effusion, hypernatremia, hypertension, and several cardiac conditions such as atrial fibrillation, congestive heart failure, and mitral regurgitation.

CTA and CT perfusion rapidly identified a left M2 occlusion. Despite the patient’s advanced age and extensive comorbidities, the neurointerventional team had to determine the most appropriate treatment strategy.

Follow the complete diagnostic work‑up and watch the full endovascular procedure, which involved distal access using a Sofia PLUS catheter and microcatheter navigation to deploy a Solitaire stent retriever.
