This case involves a 88-year-old female with atrial fibrillation, receiving oral anticoagulation, and having high blood pressure. Additionally, she has a history of breast cancer, for which she has been successfully treated. The day before her admission, she was hospitalized at another facility due to subacute ischemia of the right upper limb, and was not sufficiently anticoagulated despite her FA (INR 1.2). The reason for her admission was a clot in the right humeral artery. The following day, she was discovered with left hemiplegia and severe ischemia.
