Initial treatment with Alteplase did not improve his condition, prompting a second dose and mechanical thrombectomy due to persistent symptoms and imaging findings. Despite delays in transfer, where his NIHSS worsened upon arrival, he underwent successful thrombectomy under conscious sedation for a distal left M3 occlusion.
