In this interview, Fathallah Islim shares with Vladimir Kalousek his long-term experience treating fusiform basilar artery aneurysms. Drawing from a rare cohort of 70 patients over 16 years, they discuss clinical presentation (mostly symptomatic, with ischemic or compressive symptoms), the strong link with uncontrolled hypertension, and the challenges posed by younger patients whose aneurysms progress more rapidly.
They highlight the evolution of treatment strategies, from aggressive multi-device approaches to more cautious, staged interventions with improved braided stents, and stress the need for careful patient counselling given the risks of both disease progression and treatment complications.
