Chuan HE (Beijing) presents a challenging case of a recurrent aneurysm in a 53-year-old man with complications. The patient was initially diagnosed with an unruptured right middle cerebral artery (MCA) aneurysm, along with left internal carotid artery (ICA) occlusion and stenosis. Treatment at that time involved the placement of Web 4-3 device.
Seven months later, follow-up imaging revealed that the aneurysm was not fully occluded. Moreover, a new small sac had formed on the aneurysm, outside of the original Web system, which had migrated into the aneurysm. To address this, the surgical team performed embolization of the new sac using coils and a stent.
