Edoardo BOCCARDI presents the case of a 63-year-old woman, an agent for opera singers, who experienced a bothersome bruit, diagnosed as a sigmoid-transverse sinus dural arteriovenous fistula (DAVF). The treatment involved placing a Copernic balloon in the sinus and injecting Phil, but Phil mistakenly entered the temporal veins, causing an occlusion and leading to a tragic outcome.
