Two days before stroke onset, a 70-year-old male patient presented to ER for severe dyspnea . At admission, atrial fibrillation was diagnosed. The CT Pulmonary angiogram showed a 2 cm lesion in the right atrium. The patient was admitted to the Bicêtre Hospital (Paris) in Cardiology Unit and started s.c. anticoagulation. Two days after, the patient suffered from sudden onset: left-sided hemiplegia, drowsiness, left facial palsy, and left extinction. The MRI demonstrated right M1 and Right ICA occlusion.
