Treatment of a small communicating artery aneurysms

Last update on June 11, 2018

What are your treatment options and strategies when faced with a 44-year-old female patient who presents with a headache and, upon examination, is seen to have a series of small aneurysms (5.7 mm Acom aneurysm, left MCA aneurysm, left pericallosal aneurysm and a small [> 3 mm] left ICA )? How many, and which ones would you treat?

From a live case transmission during LINNC Paris 2018, join Prof. Jacques Moret as he and his expert panel comment on a case performed at the NEURI center (Bicêtre University Hospital, France) by Prof. Laurent Spell assisted by Drs Marta Iacobucci (Italy) and Thomas Yasuda (Brazil).

As the intervention unfolds, Jacques Moret comments on each step, from the choice of tools – including the advantages of the dedicated table the Bicêtre team employs – to the pros and cons of the strategies chosen.

Learn the reasoning behind the choice of strategy when it’s determined that the Acom aneurysm needs to be secured; and Prof Moret’s discussion about the flow-diversion attributes of the Y-stenting technique. Alternative choices are discussed as well, such as a WEB device or coiling with a balloon catheter.

See the various tips and tricks of Y-stenting, the choice of which branch to catheterize first or the length of the stent used. Learn why in this specific case that a laser cut stent is chosen as opposed to a braided stent. Listen to the discussion concerning the choice of ticagrelor and aspirin; or what DAPT regimen is employed. What imaging would you employ to confirm correct placement of the coils, Vaso-CT?

When the intervention began, a complication in imaging the aneurysm required a reversal of the projection employed – how will this play out at the conclusion of the case? Discover this and more by joining the LINNC audience and Paris expert panel here…
