This 47-year-old man with no past medical history woke up with a mild aphasia. He arrived at the hospital with an NIHSS of 1 and got an MRI five hours later that revealed a small clot. Two hours later, the patient presented a worsening of the symptoms (NIHSS of 5), but the new MRI was exactly the same. Once he arrived in the operating room, he was at an NIHSS of 7.
