A 26 years old male patient was involved in a violent car accident, and 1 month after appeared left exophthalmos with ptosis and mydriasis leading to the discovery of a carotido-cavernous fistula. Initial treatment with detachable balloon led to a major recurrence, which was finally cured with a flow-diverter stent and a liquid embolic device.
