A 65-year-old male with a history of right optic nerve section in childhood resulting in monocular vision and lumbar arthritis felt down in his bathroom with left hemiplegia and dysarthria. He was transferred to a stroke center and NIHSS was 14. The MRI showed distal emboli on the right Anterior Cerebral Artery (ACA) territory. The TOF was clearly showing the obstruction of M1 on the right side. Thrombectomy was performed 4 hours after stroke onset.
