LINNC Seminar 2024 - Americas Edition – Where the world of INR comes together!

We are back in beautiful Miami, FL, USA, at the Hilton Dadeland Hotel for the LINNC Americas 2024 Seminar for DAY 1, sharing wonderful cases and pitfalls among friends!

LINNC Seminar 2024 - Americas Edition began with a warm welcome by the seminar directors Giuseppe Lanzino, Jacques Moret, Laurent Spelle, and Vitor Mendes Pereira.

Prof. Jacques Moret started the day by presenting the first recorded case "in live conditions" which featured a challenging case of a ruptured left parietal micro AVM treated through arterial and venous access. Teaching points included distal navigation with microcatheters, venous anatomy, and liquid embolic injection in a very small nidus.

Then, for the 2nd recorded case, Prof Laurent Spelle presented a very challenging dissecting aneurysms of the left vertebral artery. The decision was made to treat it with a flow-diverting stent, which led to interesting debate with the expert panel. Dr. Stavropoula Tjoumakaris, a member of the expert panel, completed the discussion by presenting a case of similar interest.

 

 

We moved through the morning with the first stroke case recorded in live conditions presented by Dr. Jildaz Caroff: a right tandem occlusion! Head or neck first? Stenting or not? The usual questions… Yes, but so important! The expert panel helped us understand how to proceed. We also debated emerging questions: IVT or not? one or two stent retrievers? This case was followed by an interesting ACOM dysplastic aneurysm treated with a flow diverter.

After a well-deserved coffee break, we continued with a more challenging recorded aneurysm treatment: a ruptured basilar aneurysm treated with classic balloon remodeling technique with coiling. The case was also marked by severe vasospasms: What should you do, or not do?

The morning was completed with case presentations submitted by attendees: "Crossed Broca's Aphasia In The Setting Of Thrombosed Right MCA Aneurysm" by Dr. Smith Shah (USA) and "A 'NICE' case!" by Dr. Ana Villamizar (Colombia).

 

 

In the afternoon, Prof. Laurent Spelle presented two MCA bifurcation aneurysms. The first one was treated with a flow diverter after a balloon occlusion test using a balloon. Discussion focused on which branch to choose for stent deployment. The second was treated using the balloon remodeling technique with an intrasaccular device. The follow-up was surprising… Then, it was time for Stroke again with a P1 occlusion treated with ADAPT.

This led to another interesting debate with the audience!

This was followed with a recorded case of a cerebellar tentorial AVF treated with a liquid embolic agent. Understanding these lesions can be very difficult, except when Prof. Jacques Moret does the demonstration!

 

Once again, the afternoon was enriched with select cases from our attendees: "Combined Transvenous and Transarterial Embolization of Spinal Epidural Venous Fistula" by Dr. Soliman Oushy (USA) and "Pipeline Stent as a Solution for Recurrent Sigmoid Sinus DAVF" by Dr. Vera Sharashidze (USA), all of which led to a new series of high-quality roundtable discussions.

 

The day ended with a session on the dark side of INR! Professors Moret & Spelle, along some of the attendees, shared several cases that had turned into nightmares because of dramatic complications. How can we manage these or, more importantly, how can we prevent these situations from occurring in the first place? Ethical questions were also raised. A very difficult topic, but it is important that we face our past mistakes in order to learn and advance.

 

 

And now it’s time for a well-deserved cocktail and enjoy the nice weather and scenery with friends and colleagues.
