Flow remodeling case

Share your opinion of Vladimir Kalousek's clinical case concerning a 31-year-old female complaining of strong headaches. The native CT images followed by CT angio show carotid T aneurysm.

Case

A female, born in 1988, comes to the neurology department to workup for her strong headaches. The native CT images followed by CT angio show carotid T aneurysm, with a diameter over 11mm, so it was not suitable for WEB device. We opt for stent and coil strategy. A LEO stent is placed and is coiled.

On the MR control followed by DSA control, we see aneurysm regrowth with coil compaction.

So, we decide to place a flow diverter inside the LEO stent. For this case, we choose pipeline shield 3.0 x 20 to cover the inflow into stent as well.

First control angiogram after pipeline shield implantation shows a reduction of the aneurysm, with smaller filling on the neck, but no cross flow from contralateral A1/A2 segment.

 

 

 

Final control DSA 6m after pipeline shield implantation shows occlusion of the aneurysm. We flow remodeling so the ACOM segment is filled through the right side, without filling of the aneurysm.
