Discover this case from Vladimir Kalousek of a 60 years-old male patient presenting with left MCA trifurcation.
Patient born in 1958, male, came to outside hospital due to psychiatric problems, otherwise good health.
On the workup it was found that he has an aneurysm on left MCA trifurcation, with dimensions seen on the images.
To patient and his family both endovascular and neurosurgical options were presented.
They opted for endovascular treatment, for which we have planned, placing a web device, dimensions of WEB SL 7x4.
Patient was placed before the procedure per our protocol on double antiagregation therapy for eventual bail out procedure.
Our setup consisted of 8f sheath short, neuron max 8 f 90 cm, navien 5f 125 cm, and via 21 micro.
Web was placed in the aneurysm, with for us optimal positioning regarding the neck portion of web towards superior and inferior branches.
We have our protocol in which we wait for 15min for eventual acute thrombosis which have not been seen.
Patient was woken up, and neurological status was excellent.
He will be on double antiagregation for one month.
Follow up DSA is planned.
