A 75-year-old hypertensive smoker presents with a Fisher Grade 4 hemorrhage and left-sided deficits. Discover how a combination of WEB device and tailored access strategy offered a safe, efficient solution in this challenging wide-necked bifurcation aneurysm.

Case presentation

 

History:

  • 75 YO male
  • Hypertension, active smoker, poor medical follow-up

Symptoms:

  • Severe headaches and dizziness
  • Then found on the ground by his son

Physical exam:

  • GCS 14
  • Paresis of the left limbs
  • Left hemineglect

 

Fisher Grade 4 hemorrhage

  

Irregular+bleb+spasm

  

100% usage of Sim&Size for aneurysmal embolization

Day 4 post-op

  

Access materials:

  • Femoral access
  • Neuronmax (by exchange technique) + Vecta 71 + Headway 27

Medication:

  • Heparin intra-op: Yes (standard practice).
  • Antiplatelets post-op:
    • WEB only → 30 days of aspirin.
    • WEB + stent → DAPT required.

Discussion & Conclusion:

  • The WEB device offers a promising intrasaccular alternative to traditional coiling, particularly in wide-necked bifurcation aneurysms
  • Procedure was completed successfully with no intra- or post-procedural complications
  • This demonstrates feasibility and safety of the WEB device in acute aneurysmal settings
  • Compared to conventional coiling, the WEB may provide:
    • Shorter procedure times
    • Less radiation to patient and surgeon
  • Continued follow-up and imaging will be essential to assess long-term durability and occlusion status
