Carotid revascularization for symptomatic thrombus after failure of medical therapy

Discover the case of a 67 year-old male who presented with stroke to an outside facility in November 2021: he was found with carotid stenosis with intraluminal thrombus, and started on apixaban. However 2 months later, he was diagnosed with COVID-19 and clinical deterioration. Physical examination revealed a severe aphasia, a right hemianopsia, a right facial palsy and a right hemiplegia. CTA displayed an eccentric mural thrombus in left CCA/ICA. After failure of medical therapy, decision was made to proceed with endovascular treatment using flow reversal.
